Cellular Hematology PT specimens are comprised of whole blood and Coagulation PT specimens are comprised of plasma. Please complete the app. Submitting a completed questionnaire as soon possible will enable us to process your application expeditiously and ensure the laboratory is enrolled in PT for the next scheduled test event, if appropriate.
