The Maryland Criminal History Check app enables the state to provide the criminal history record based upon an individual's consent.

An applicant using this form can provide their full information including their full name, address, drivers license number, social security number, gender, race, and date of birth. The agency requesting the criminal history records must also provide their full information, authorization number, and position the applicant applied for.
Out of state applicants must provide a fingerprint card, along with the required electronic form. In state applicants must fill out the complete form, be fingerprinted a state certified location and return it to the Criminal Justice Information System. The reason for the requested criminal history check must also be provided by the agency.
