The Kentucky Criminal Background Request app will allow for criminal background requests to be made on an individual.

An individual or agency must provide the individual's full information including their full name, address, social security number, and date of birth. The reason for the criminal history request must be noted including employment, criminal investigation, licensing, housing, or volunteer.
The full information of the requester must also be provided including contact information and signature. A fee must be provided to the Kentucky State Treasurer unless the requester of the criminal record is a criminal justice agency.
