The New Account Request Form app allows a contractor to request an account from the State Treasurer’s Office.

When applying for an account for construction project account, the contractor must first apply for a new account. All details about the contractor can be provided through this app to the State Treasurer’s Office for an account to be created.
The contractor can provide its full name, address, and contact information through the app. The Federal tax ID number and title of the person applying are also able to be submitted through the app.
