The Disabled Veteran Business Enterprise Declarations Form app will allow for all owners of subcontracted companies to declare all supplies and materials provided to a construction project.

The details of the certified DVBE will all be listed including the name of the company, DVBE reference number, and the solicitation or contract number. The full description of all materials, supplies, services or equipment provided by the DVBE company must be listed.
The Disabled Veteran Business Enterprise will declare whether they are a broker or agent, as defined by the Military or Veterans Code, of the materials, supplies, services or equipment described in the form. If they are acting as a broker or agent, the firm name of who they are supplying should be provided. If the DVBE is not a broker and rents the equipment and supplies to the firm, they must own at least 51% of the company and provide certification.
