A contractor can be deemed non-responsible for trustworthiness, requisite quality, fitness, and capacity. The details about the contractor and project should be provided including the location, contract number, project name, contractor, and contractor license number. The specified and actual length of time for the construction project should both be noted.
The officials will provide details about any liquidated damages assessed, any letters issued to the contractors, the cost overrun amount, and any other description of why the contractor has been deemed non-responsible. Any notices and hearings that have involved the contractor should be provided. If a recommendation of delisting is recommended this should also be noted on the form.
