U.S. Department of Transportation (DOT) Alcohol Testing Form

Department of Transportation (DOT) Alcohol Testing Form is a four-step form that can be filled out by both an alcohol testing technician, as well as an employee subjected to DOT alcohol testing. This app takes in inforamtion including an employee name, their social security number or employee ID number, the address, telephone number, and the reason for testing. An alcohol test may be administered for a range of reasons including random testing, reasonable suspicion, after an accident, when an employee is returning to duty, as a follow-up to a previous incident or test, or as pre-employment testing. There are different types of tests that can be administered including breath alcohol testing, or a saliva test. The app allows a technician to note the test number, the testing device, the device serial number, activation and reading times, and results, specifically noting an alcohol concentration. The technician administering the test can not other remarks (such as an odor of alcohol or other relevant information such as collection site information). DOT employees are subject to regulations concerning safety in the workplace, and as a part of these regulations, drug and alcohol testing are authorized by the federal government in safety-sensitive positions, as well as other positions, such as those where an operator has a commercial driver's license. This app makes documenting testing results a breeze.
