Maternity Adoption Leave Curtailment Notice (UK)
This form should be completed by an employee who wishes to curtail (i.e.

shorten) their maternity/adoption leave (and pay) to access Shared Parental Leave (SPL). Applicants should use this notice to curtail their maternity/adoption leave and pay at a future date. Users should ideally submit Form 2: Application for Shared Parental Leave at the same time as this app. The Maternity Adoption Leave Curtailment Notice (UK) app also features a link to the Shared Parental Leave Policy (SPL) so applicants can read over the rules and regulations. In the app, applicants will need to provide: employee name, employee number, contact information, manager information, which option the user would like to curtail (maternity leave and pay or adoption leave and pay, date of birth/placement, and a signature.
