The CSA Transportation Claim Form Loss and Damage app allows users to submit claims for carrier and shipment items that have been lost, stolen, or damaged. The claimant can provide shipment information, shipper's name, consignee's name, point shipped from, final destination, and description of items claimed. The claimant will also submit damaged goods information, including indicating the quantity, description, landed or invoice cost. After app users file a claim, the claims office can then review the submission and determine a liability to settle the claim in a quick and fair manner.
