California Veterinary Certificate of Quarantine (Form CDPH 103)
Licensed veterinarians in California should use the California Veterinary Certificate of Quarantine app, provided by the California Department of Public Health, to help navigate the animal quarantine process for obtaining a health certificate for an imported wild animal that an owner would like to keep as a pet.

This app should be used to demonstrate that an imported wild animal has had a proper quarantine period (or observation period) at an animal quarantine facility, as well as tests for infectious diseases, including a required tuberculin test, upon its arrival in California. Other health-related concerns, which may impact the health certificate process or put the public at risk, will also be documented in this app. Required information must be completed by the owner and a veterinarian who has physically examined the animal and can confirm the animal is free of contagious or infectious diseases, such as the rabies virus. The veterinarian will sign this form, attesting that the pet has been quarantined for the proper quarantine period and eligible for a health certificate.
