Quarantine Form for Animal Exposure

This app is helpful in establishing the need for a rabies quarantine for the owner or handler of a domestic animal that may be exposed to a rabid animal. Keeping a pet or animal in quarantine for the duration of the rabies incubation period can help in rabies control procedures. An animal's owner is required to maintain an at home quarantine of their pet for observation of clinical signs of the rabies virus for at least ten days. This app will provide health department officers the details they need to share with the owners to quarantine an animal. The owner will be required to provide proof the pet has a rabies vaccination certificate, agrees to a quarantine period with limited exposure to other animals, and agrees that the quarantined animal will only be allowed outside if it is leashed for the observation period. The animal's veterinarian is made aware of the quarantine through this app. The owner should observe the animal for any abnormal behavior or clinical signs of rabies after the animal exposure or bite as this could become a public health concern. If there are any clinical signs of rabies during the observation period, an exam by a veterinarian may be required, and the owner must contact the health department.
