The child will need to complete basic information about themselves, as well as provide proof of age, such as a birth certificate, age certificate or driver's license. A parent or legal guardian will need to sign the app to acknowledge that they are aware the child is applying for a job. The employer will need to complete a section outlining the hours the child will be working, acknowledging that the position will not conflict with school hours, and agree to abide by federal child labor laws. The employer must also assure that they are not exposing the child to hazardous occupations, they are paying them at least the minimum wage and not requiring them to work during school hours in the school year. Additionally, a physician will need to complete part of the app, certifying the child is the age listed on the application, and is healthy enough to perform the tasks of the job. So long as the physician, the employer, and parent or legal guardian sign the application for a permit, the child should be permitted to work.
