This app for NIH Form 2683 will allow a user, most likely a laboratory manager, safety supervisor, or other laboratory personnel who have most recently been in the facilities, to certify that the areas have been decontaminated and are without hazardous materials. For each hazardous material listed, the user will have an option to claim that the material, chemical, radiological, biological or oil/heavy metal (such as lead-based paint), has either been in the space and decontaminated, not in the space at any point, or currently is in the space. Each type of hazardous material will have listed reference information for instructions on how to decontaminate the area. The user will need to certify, by signature, that the lab area is either free from hazardous materials or has been decontaminated. For some hazardous materials, an inspection from an agency, such as the Radiation Safety Branch, may be required for clearance.
