With the form, the app user will record the household number, age of children, vaccination status, and any known reasons reported for not being vaccinated. The app includes a set of useful guidelines for completing the household assessment and protocol for follow-up and handling cases of non-vaccinated children. The app user will find helpful coding for responses regarding reasons for not being vaccinated such as located too far from the clinic, already had routine immunization, or opposed to vaccination. Sheets are submitted to the campaign coordinator to better inform immunization programs to prevent an outbreak of measles, and, ultimately, promote public health in the region.
