Because a highly contagious virus such as that causing measles or mumps puts a school population at risk, university health services require an immunization form from all students for disease control purposes. The Measles, Mumps and Rubella Form app is a useful tool for students to share MMR vaccine records with student health services. The immunization form is to be completed by university students and submitted to student health services before a student arrives on campus. The form must be completed by a health care provider to indicate the date on which a student was vaccinated for MMR and the dose of each vaccination. If the students did not receive the MMR vaccination, the healthcare provider can record when the individual vaccinations for measles, mumps, and rubella were administered.
