The New Jersey Investigation Outbreak Intake Record app assists health investigators in collecting initial information from individuals calling to report infectious disease outbreaks or other public health events. The health investigator will note the contact information and details for the person who reported the outbreak such as the local health department, school nurse, or a primary care provider. The health investigator will describe the event and classify it as gastrointestinal illness, respiratory infection, rash, or other and note the location such as school or health care facility. For the common symptoms reported, the health investigator will record the first date of symptom onset, as well as the most recent date of symptom onset, and the number of persons reporting for each. Finally, the investigator will identify risk factors such as travel to a place known to have an illness and exposure risk to other persons, as well as the disease control measures recommended and implemented.
