The form is required to be completed for all fixed or mobile helicopter fueling facilities operated by the government, or for fixed facilities operated by a vendor and that are located on government land. The vendor is responsible for inspecting vendor-owned facilities located on government land, or government-owned facilities for which the vendor is contractually responsible (for example, the vendor is required to maintain and fill a remote fuel cache).
