The purpose of the Helicopter Information Sheet (HCM-6) mobile app is to provide the Helibase Manager and other air operations branch personnel with information concerning the helicopter, the Pilot, and the vendor’s ground crew (driver/mechanic) assigned to multiple-aircraft helibases.

It summarizes most, if not all information relating to each individual helicopter operation at a helibase, thus relieving the Helibase Manager from having to obtain this information at various times over the course of the incident or project. This form is required for large fire operations and optional for projects until requested. The Helicopter Manager for both exclusive-use contracts and CWN is responsible for completing the form prior to or immediately after arrival at an incident or project helibase. The Helibase Manager is responsible for obtaining the Information Sheet immediately upon arrival of a helicopter at an incident or project.
