Confirmation of Remediation Checklist (Canada)

The mobile form includes fields to document items such as site location and contact information, site professional, limited remediation, full property remediation, and remediation criteria determinations. Once the mobile app is filled out a secure PDF is generated. The PDF is easy to share, and a copy is stored in your account for your soil and groundwater remediation records. You can also use the App Builder to personalize and customize this mobile form. All relevant Sections of this app must be completed and must accompany the Confirmation of Remediation Report. If there are no impacts identified to third-party properties Section 4 must not be completed. The Confirmation of Remediation Checklist should be submitted in conjunction with any other documentation for site closure; such as FRM-700, Record of Site Condition or FRM-701, Declaration of Property Condition. Signatures on this app are required from the managing Site Professional. Separate checklists are required to be submitted for each impacted property. All regulatory protocols must be followed and must be completed separately for each property. This means that a source property and an impacted third-party property must have all documents filed separately. Once the source property or impacted third party property is identified by the checkbox below, all subsequent reference on this app is to that site owner.
